My Group Life Benefits

Eligibility:

All full-time employees who work at least thirty (30) hours per week are eligible for Basic Life and AD&D coverage on the first of the month following 6 months.

EMPLOYER-PAID GROUP TERM LIFE AND ACCIDENTAL DEATH & DISMEMBERMENT

You receive Term Life Insurance equal to 1 time your basic annual earnings, with a maximum of $500,000.  Benefits reduce to 65% at age 65, to 50% at age 70.

My Long Term Disability Benefits

Gene B. Glick Company provides employees Long Term Disability insurance.  The entirety of the cost of this benefit is paid for by Gene B. Glick Company.

Eligibility

All full-time employees who work at least thirty (30) hours per week receive long-term disability coverage the first of the month following 6 months of employment.

The benefit is equal to 60% of earnings up to $7,500 monthly.  Benefits begin on the 181st day absent for illness or injury, and can continue until the employee attains Social Security normal retirement age.

My Voluntary Short Term Disability Benefits

Gene B. Glick Company offers employees Short Term Disability coverage for employees.  The majority of the cost of this benefit is paid for by Gene B. Glick Company.

Eligibility

All full-time employees who work at least thirty (30) hours per week are eligible for short-term disability coverage the first of the month following 6 months of employment.

The benefit is equal to 66.67% of earnings up to $3,200 weekly.  Benefits begin on the 15th day absent for illness or injury, and can continue for up to 24 weeks.

My Voluntary Life Insurance Benefits

Eligibility

All full-time employees who work at least thirty (30) hours per week are eligible for voluntary life coverage on the first of the month following 6 months.

Eligible Dependents Include:

Your legal spouse.  Your legal child(ren), Child(ren) includes your natural, adopted, and step child(ren) or any child whom you have legal custody over.

Benefits:  Benefits are elected in increments.  Elected benefits exceeding the Guarantee Issue are subject to medical underwriting.  New Hires must enroll in these benefits during their initial eligibility period.  Late Applicants can enroll at anytime, subject to medical underwriting.

Employees may elect amounts of voluntary life in increments of $5,000 up to $500,000, with a minimum benefit of $20,000.  Benefits reduce to 65% at age 65, and to 50% at age 70.

Spouses may elect amounts of voluntary life in increments of $5,000 up to $150,000 or 50% of the employee’s benefit.  Benefits reduce to 65% at age 65, and to 50% at age 70.

Children may elect amounts of voluntary life in increments of $500 up to $10,000, with a minimum benefit of $500.

My Flexible Spending Account Benefits

Flexible Spending Accounts

If you think saving money on health care and dependent care is hard, think again! Flexible Spending Accounts give you a valuable tax break. They allow you to pay eligible expenses tax free – helping to make your income go farther. You can contribute up to $2,800 a year to a health care spending account and/or $5,000 a year to a dependent care spending account (minimum annual contribution of $260 for each account.)

Eligible Health Care FSA Expenses

You can be reimbursed up to $3,050 a year for eligible out-of-pocket expenses not covered by your medical, dental and vision plans such as deductible and copays. You will need a prescription from your physician to utilize funds from your Health Care FSA to purchase over-the-counter medications.  You can roll over up to $610 of your Health Care FSA dollars from year-to-year.

Eligible Dependent Care FSA Expenses (All Eligible Employees)

This account allows employees to set aside pre-tax dollars for dependent care expenses for children under age 13, and/or dependents incapable of self-care (for example: disabled children over the age of 13, or expenses associated with your dependent parents if they live with you). Employees can contribute up to a maximum of $5,000 each year if their income tax return status is single, or married filing jointly; $2,500 each year if married filing separately. Refer to IRS Publication 503 for eligible expenses and limitations for the dependent care FSA.

The Tax Advantage

Because your contributions are deducted on a pre-tax basis, you pay less in taxes. The savings are at least 25% for most individuals.  For more information, visit the IRS’ website.
